That mask is actually "attached" to the hair of the model, if you want to just use that one (if you do not already have a hair/eye mod), it is Ren's Beauty Pack.

is there a way to create the same mask separately from the hair?

I use this http://www.tesnexus.com/downloads/file.php?id=3730, it looks pretty good with the Dark Brotherhood Armor.

You can also use the Snow Dragon Temple mod, as the ninja armor comes with a lower mask and hood (Seperate). The fact that you can wear the mask with a helmet makes it pretty cool, too.
